Whether you’re just beginning to explore your options or ready to move forward, our team is ready to support you with kindness, clarity, and care.
What to Expect
We know that no two journeys are the same. When you’re ready to begin the conversation, we’ll start by learning more about your needs, answering your questions, and helping you understand what care might look like at Odd Fellow-Rebekah Home. From there, we’ll guide you through each step from touring our home to getting everything in place for admission.
Some of the things we’ll talk about:
- Your or your loved one’s medical needs and preferences
- Level of care and support desired
- What daily life looks like here
- Room availability and what to expect upon move-in
- Financial options, insurance, and coverage
What You'll Need
When you’re ready to move forward, we’ll ask for a few important documents. These help us ensure that we’re fully prepared to meet your needs and follow all legal and medical protocols.
You’ll want to gather:
Identification & Insurance
- Photo ID
- Social Security Card
- Insurance Cards
- Medicare and/or Medicaid Cards
Legal & Medical Paperwork
- Power of Attorney Documentation (Healthcare & Financial)
- Advance Healthcare Directives (Living Will, POLST forms, etc.)
